What is Heart Failure v2.0?

The new HF v2.0 is an operational model for facilities to plan and organize the delivery of care in a systematic manner that supports the adherence to evidence-based guidelines and quality initiatives. SPCP accreditation incorporates the principles of Value-based Purchasing (VBP) to help facilities with achieving optimal financial, quality and patient satisfaction outcomes.

Why get Accredited?

SCPC’s approach to HF is radically different from others that set specifications and measure compliance. SCPC’s Review Specialists take a collaborative approach by providing feedback, education and resources to assist the facility in addressing gaps and improving processes.

HF v2.0 is for hospitals interested in:

  • Reducing HF readmissions
  • Managing HF more effectively in the field and the ED to reduce delays in treatment
  • Improving patient satisfaction scores
  • Engaging the care coordination team to effectively and efficiently manage HF patients across all transitions
  • Potentially reduce CMS penalties associated with VBP

New Technologies.

HF v2.0 has a new dashboard and the software helps you see metrics, understand key issues and opportunities, and improve across the organization. And our reports are specific to your data, your needs, your processes, because we’ve been in your hospital and we know its unique needs.

“No organization innovates
like the Society.”

  Wil Mick CEO SCPC

Listen to Wil's webcast from earlier this year, when he outlined the Society, it's goals and his vision to help the Board achieve a level of excellence for the Society.

Uniquely qualified.

SCPC is uniquely qualified to deliver this solution, because we are neutral and objective in our approach, but collaborative in our delivery. That means facilities that commit to HF v2.0 will get our promise: we’re prepared to educate and update our HF community when the standard measures shift, or the CMS measures change … because we’re already anticipating changes and HF v2.0 has been built to adapt.

Q+A: The Clinical View

Donna Hunn
HF Service Line Specialist

This product looks comprehensive and includes a new software back end and reports. Is the Society getting away from paper-based binders?

Yes, we are going to a totally online submission process, thus eliminating the creation and management of large binders. It is also the “green” thing to do!

Q+A: The Technology View

Abe Joseph
Director, Product Management

The new HF tool has software that can produce reports in a dashboard that Accredited hospitals can review. Why are you going in that direction?

As a leading accrediting body, we believe that our accredited facilities must have the ability to track their performance against the metrics identified within the accreditation protocols we have developed for cardiovascular patient care. We are launching this new capability with our new HF tool so that accredited hospitals can improve patient outcomes and we know how well our accredited facilities are performing.

Q+A: The Physician Point of View

Dr. Phil Levy
SCPC Board Member
HF Essential Component Committee Co-chairman

What information, or data, do you see in your hospital that the new Heart Failure tool should address?

Process measures such as the proportion of eligible patients who receive beneficial medications not tracked by CMS metrics (i.e., beta-blockers, aldosterone antagonists, and isorbide dinitrate/hydralazine), the proportion who receive and attend a post-discharge appointment, and the use of palliative care consultation for end-stage patients will be of tremendous value. Additionally, the inclusion of outcome measures beyond simple mortality or recidivism along with more extensive breakdown of the time frames for their occurrence will be important.
